Itchy skin and patchy fur on a terrier mix?

I have a terrier mix (believed to be anyway...he's a mutt we rescued as a puppy) who constantly has itchy skin and patches of thinning fur. He's been to the vet multiple times and has been given antibiotics, antifungals, cortisone, you name it. He gets better for a few weeks, but always ends back up in this state. I'm at a loss for what to do. Any suggestions?

Check his flea treatments are up to date.

If they are, then it could be diet related so either tests to confirm what he's reacting to, or an elimination diet, ie something like salmon and potato as he's not likely to have had this before, if his itching stops, then add one food per month to see how he reacts to it.

Another possibility is an environmental allergen (grass, pollen etc)

Also look at the cleaning products you use to see if any of these alter his condition when you change them

Have you had his hormone levels checked? Also have you tried a antihistamine to help stop the itch scratch cyle?
