I love fishing, hiking, and dogs. I'd hate for this to happen to my dogs or any dogs or animals period. I know people are halfway blaming the fishermen for leaving hooks around, but really it's not always our fault like marley and Phil said.
Sometimes your line catches on a tree, log, seaweed etc, and the only way to get it loose is to snap your line, which leaves your hook in whatever it was stuck in and it also leaves some line usually.
I'd NEVER leave hooks OR line out of pure laziness, and I don't think most fishermen do this either. Some do sadly, which does cause problems... but I don't think most fishermen do this.
Sad story, but not necessarily preventable.
