What the settings should mean:

Every visit to the page
IE checks to see if the webpage in the cache is the same as the one on the site. If not, it downloads the latest version.

Every time you start Internet Explorer
IE checks to see if the webpage in the cache is the same as the one on the site, but only when you start it up. If not, it downloads the latest version. If you never close IE, it will never get the most recent version

God knows!! IE does what it wants, perhaps causing random slowdowns and crashes. IE does what microsoft programmed it to do, sometimes displaying the most recent version, otherwise displaying an older version of the webpage. You'll never really know

Never
IE always uses the cached version where possible. Not even sure why this setting is of any value ???
OK. That was how it was before ISPs started using web caches. If say customer A downloaded a page from a site in the States and half an hour later, customer B requested the same page, the ISP would supply the very same page to customer B, by using the one they stored in their web cache. This means that the ISP saves bandwidth costs and customer B gets an ultra quick download. The disadvantage is that the page might have changed in the meantime, but neither customer A, B or the ISP would know.

In order to ensure that the most recent page is being displayed, in IE, you need to empty your temporary internet files folder and refresh the page using CTRL-Refresh (hold down the CTRL key and click on Refresh or hold down the CTRL key and press F5).
