Definitely sounds like the mange to me, I used to breed guinea pigs and a few of them got it, because I'm diabetic and know how to inject, they gave me the medication to inject them, it soon cleared up. The problem is mange causes stress and stress will kill a guinea pig very quickly, so I would go back to the vet asap.
