I had a dog with chronic kidney failure and I fed him a home made diet, as he had lots of food intolerances and couldn't cope with the prescription diet the vets offered us. Hills kd is full of cereals which are not necessarily a good thing for kidney patients.

I fed him on 50% white rice and 50% fatty lamb or boneless white fish or chicken. Sometimes I used sweet potato instead of rice.

basically you want to give 50% carbs and avoid too much phosphorous and potassium. Avoid bones and bony fish (including tinned), green leafy veg, beef, eggs and gluten. Low protein is often advised but if you feed good quality protein from natural sources it will be within the safe levels. I fed this diet to my dog with my vet's blessing. IME prescription diets are often full of low quality ingredients (just read the labels!) and I wanted to know exactly what was going in my lad's body.

Sorry no idea. Depends on your dog and his activity levels etc. I would trial and error when it comes to amounts.
